    both A & B should be out ..because avoiding duplication of facilities will not cause wasteful use of resources..rather its just the oppposite

    C, D, E three go out as the phrase should be a reason why they are wasteful. c,d,e suggest an act to be done. And between A & B, avoidance suggests more a reason (people’s avoidance of duplicate facilities).
    I go with A.

    However, in above sentence “alone” really seems out of place…If the phrase on the grounds of is being used, then using “on the grounds of avoiding duplicated facilities” would have been enough..alone is signifying that it is being stressed here that this alone can be the reason not to build large new hospitals in bistate area.
    That makes it skeptical.

    now, moving to E:
    Building large new hospitals in the bistate area would constitute a wasteful use of resources, if only because the duplication of facilities should be avoided.

    This clarifies that “if duplication of facilties is the lone criterion, then building of large new hospitals would be a waste of resources.”

    That makes it “E”…but definitely a very typical question. It is more about not selecting the other choices rather than selecting “E” at first place.

    “if only because” is wrong usage. This rules out “E”.

    If you omit “if only” in sentence E, then sentence E would make the most sense.

    no kramer….sense changes by omitting “ïf only”
    because omitting generalizes the statement but that is specific here and not a general one.

    A:verbose and meaning not very clear
    C:solely is wrong… writer mentions it as one of the reasons and not the only reason
    D:while does not belong here.
    E:meaning is clear, but it is verbose.

    I go with A.

    In this question “Should” is too definitive and instructive and does not convey the correct message . Hence i would eliminate C, D and E.

    Its not that duplicate facilities “should” be avoided. But its one of the basis.
    Between A and B, i would eliminate B because “Grounds of” .. “alone” do not go together (Plural / singular). Also, the use of simple continuous “avoiding” is incorrect here.

    Hence correct answer in my opinion is A.

    Someone please confirm.
